Intensive Case Management(ICM)
Intensive Case Management (ICM) at Wellness Solutions Counseling provides structured, hands‑on support for clients who need more than traditional therapy to stabilize their lives, access resources, and move toward long‑term wellness. This service is designed for individuals facing complex challenges such as housing instability, mental health concerns, justice‑system involvement, family conflict, or major life transitions.
ICM blends clinical insight with practical, real‑world support. Clients receive consistent check‑ins, collaborative goal‑setting, and direct assistance navigating community, medical, legal, and government systems. The focus is on reducing barriers, strengthening stability, and helping clients build the skills and supports needed to thrive.
What Is Included In ICM

Who Is ICM for

Our case management is trauma‑informed, collaborative, and culturally responsive. We meet clients where they are, walk with them through difficult systems, and help them build stability step by step. The goal is not just short‑term problem‑solving, but long‑term empowerment.
Intensive Case Management Fees & Packages
Intensive Case Management is available as an hourly service or as a monthly support package.
Hourly sessions are billed at $250, and monthly packages range from $1000–$1200, depending on the level of support, frequency of contact, and coordination needed. Allowing you to choose the structure that best fits yourt needs and goals.
